Someone from the courthouse brought me this letter and I am suppose to respond in 10 days. Should I just pay the amount, I am not sure why insurance didn't pay!

You should PROBABLY file an answer. I say PROBABLY because if there is some problem with venue, you can't complain after you've filed your answer - you're stuck in that court.
We need more information. You should really go see a lawyer.
