I was involved in auto accident 10/12/2010
my health insurance took care of the bulk for all treatments including surgery (11/18/2011).
The party at fault will not make me a settlement offer yet.
Reason - a lien by my health insurance.
What should I expect from here on?

If you're not represented, don't expect much. The insurance companies will deal with each other, because there's no one looking out for you. If you are represented, then your attorney should try to negotiate with the health insurance company to reduce the lien.
